Multitrauma with cerebral edema
Findings:
CT brain window image demonstrates a subdural hematoma over the right cerebral convexity with midline shift, intraventricular hemorrhage, and numerous hemorrhagic shear injuries within the right basal ganglia. There is overlying scalp soft tissue swelling.
CT bone window image demonstrates comminuted displaced fractures of the right temporal parietal region, including a transverse component through the right temporal bone. There is gas within the right sigmoid sinus. Orbital gas is present with small orbital hematoma. There is opacification of the sphenoid sinus with a central skull base fracture through the sella incompletely included.
VRT image demonstrates absent enhancement of the distal supraclinoid internal carotids, with hazy reduced enhancement in the basilar artery. The source images demonstrate absent intracranial arterial and venous enhancement above the level of the cavernous sinuses.
