Arachnoiditis

Findings:

Sagittal CT reconstruction demonstrates postoperative changes of L3 through L5 laminectomy with advanced multilevel degenerative disc disease and vacuum disc phenomenon. Multiple MRI images demonstrate amorphous overall hypointense material within the thecal sac at the L4 level with individual nerve roots not well distinguished. Mild enhancement is present in this region after gadolinium administration, with heterogeneous signal in the distal thecal sac. Mild linear enhancement of nerve roots of the cauda equina is identified. The axial images demonstrate heterogeneous signal in the left paravertebral region extending into the paravertebral musculature. A focus of T2 hyperintensity extends from the left facet joint causing mild mass effect on the thecal sac.
