Toxoplasmosis

Findings:
Multiple ring enhancing lesions are present throughout both cerebral hemispheres, with associated marked edema.

Differential diagnosis:
mets, toxo, lymphoma, TB, abscess

Discussion:
  -10% of AIDS pts., up to 40% of CNS infections in AIDS
  -up to 70% seropositivity in general population
  -clinical: H/A, fever, lethargy, focal neurologic deficit
  -necrotizing encephalitis with thin walled abscess formation
  -1-4 cm lesions, small lesions solid enhancement, larger ring enhancing
  -locations: BG, WM, CM jcn
  -major ddx in AIDS= lymphoma
        -helpful distinguishing factors: no spread across corpus callosum, larger number of small lesions, response to Abx
