Multiple Trauma
Findings:
basilar skull fractures which extend through the carotid
canals, R temporal epidural hematoma, contusions, SAH/IVH, bilateral
ICA dissection with near occlusion on the left, active extrav from L internal
maxillary branches- embolized. Injection of RCCA shows active extravasation
from middle meningeal artery.
Discussion:
Fractures which extend through the carotid canals have
a high association with vascular injuries such as dissection. In the setting
of massive facial trauma and skull base fractures, arterial extravasation
at the time of angiography is not uncommon, and may be successfully treated
by embolization.
