Findings:

Multiple axial T1 postcontrast MR images demonstrate abnormal leptomeningeal enhancement along the superior cerebellar sulci as well as mild coating of the brainstem.

nSarcoid, TB, mets, meningitis, (LCH)

 

nClinical/Path

 

n3% with no systemic disease, 5% clinically, 15% at autopsy

 

n50% of CNS involvement is subclinical

 

nNoncaseating granulomas, intraaxial or extraaxial, perivascular, vasculitis/infarcts

 

nImaging

 

nMR- diffuse or focal leptomeningeal/pachymeningeal enhancement, nonspecific white matter lesions simulate MS,

 

focal mass, basal meningeal, pituitary/infundibulum, cranial nerve enhancement, hydrocephalus
