TB

TB adenitis

Findings:

Axial contrast enhanced neck CT images demonstrate multiple centrally necrotic lymph nodes in the right neck and supraclavicular region, with mid displacement of the thyroid gland. Chest CT image demonstrates diffuse bilateral tiny lung nodules.

Differential Diagnosis:

Suppurative adenopathy from bacterial or mycobacterial agent, nodal metastases, lymphoma

Discussion:

nExtrapulmonary TB uncommon (4.5%)
nH/N manifestations only 12% of extrapulmonary TB, most of these are adenitis
nPrimary ddx would still be lymphoma in absence of pulmonary findings
nTB nodes may be variable density, often necrotic/low density, lymphoma less commonly necrotic unless aggressive/high grade
