Central Neurocytoma
Findings:
A large heterogeneously enhancing intraventricular mass is present with associated obstructive hydrocephalus on this series of MR images. The mass has heterogeneous signal characteristics with zones of increased and decreased FLAIR signal and several complex cystic zones.
Differential Diagnosis:
Central neurocytoma, Subependymoma, SGCA, metastasis, CPP, meningioma, ependymoma
Discussion:
This is typically an intraventricular mass attached to the septum pellucidum and is often a complex cystic and solid mass. Calcification is common in 50-70% and hemorrhage is rare. Hydrocephalus is common. The masses show strong enhancement on CT and MR. The tumors are histologically benign and surgical resection can be curative although recurrence may develop. 75% are seen in young adults 20-40 with no sex predilection. Extraventricular lesions with neurocytic features have a more aggressive course and poorer prognosis.
