Intradural Disc Herniation

Findings:

Multiple axial images from CT myelogram demonstrate an irregular intradural lesion on the left which is inseparable from the left ventral nerve root and causes mild mass effect on the left lateral spinal cord. The structure is generally contiguous with the region of degenerative disc disease and uncovertebral hypertrophy, with severe bony narrowing of the neural foramen.

Differential Diagnosis/Discussion:

The irregular morphology, partial calcification, and contiguity with adjacent disc disease raises the high likelihood that this is a disc extrusion that has penetrated the dura. The irregular morphology would make more common causes of intradural extramedullary masses such as schwannoma or meningioma less likely.
