Susac Syndrome

Findings:

Multiple MR images demonstrate small ovoid signal abnormalities somewhat preferentially involve central fibers of the corpus callosum. Some of these demonstrate diffusion restriction. Scattered other somewhat ovoid lesions are present within the deep frontal white matter and the basal ganglia.

Differential Diagnosis:

The presence of ovoid periventricular white matter signal abnormalities typically raises concern for demyelinating process such as multiple sclerosis. However, lesions of multiple sclerosis typically involve the undersurface and peripheral aspects of the corpus callosum, while vasculitis such as Susac Syndrome tend to involve the central fibers of the corpus callosum. Clinical features also tend to separate MS from Susac.
