T4 squamous cell carcinoma, larynx
Findings:
A poorly defined irregular enhancing mass involves the right vocal cord and subglottic region, crossing midline at the anterior commissure and penetrating the thyrohyoid membrane, invading the strap muscles. The mass is also inseparable from the right aryepiglottic fold and the cricoid cartilage. The bone windows demonstrate destruction of the thyroid cartilage anteriorly.
Differential Diagnosis:
Cancer is the top differential diagnosis, of which squamous cell would be by far most common. Less likely possibilities include lymphoma, sarcoma, and pseudotumor.
