Recurrent Epidermoid

Findings:

Postoperative findings of right mastoidectomy are present with a large nonenhancing zone within the surgical defect demonstrating prominent restricted diffusion.

Differential Diagnosis/Discussion:

When reviewing postoperative patients, it is very important to determine the histology of previous lesion resection. Blood products within a resection site may cause diffusion hyperintensities, but in this case there was a known epidermoid resected and therefore this focus of restricted diffusion represents recurrent epidermoid.
