Neurosarcoidosis

Findings:

Multiple MR images demonstrate extensive nodular leptomeningeal enhancement which also coats the brainstem. The process has a slightly basal distribution. Additional nodular leptomeningeal enhancement covers the distal thoracic cord. Small zones of surrounding vasogenic edema are present within the frontal lobes and a few patchy subcortical signal alterations are present. The process is not associated with restricted diffusion.

Differential Diagnosis:

The differential diagnosis of leptomeningeal enhancement includes infectious and noninfectious inflammatory processes, malignancy, and occasionally seen with seizures but usually isolated to the meninges overlying the seizure focus.
