Venolymphatic Malformation

Findings:

A well-defined extraconal lesion is present in the medial left orbit which is inseparable from the extraocular muscles. The lesion demonstrates no significant enhancement. On the axial images, a subtle fluid fluid level is seen within the lesion.

Differential Diagnosis/Discussion:

The differential diagnosis of a well-defined intraconal orbital mass includes hemangioma, lymphoma, and venolymphatic malformation. Intense enhancement would be expected for hemangioma. Lymphoma would also be homogeneous and enhance. The presence of a fluid fluid level indicates a cystic lesion with slow flowing internal blood or other products that indicate a venolymphatic malformation.
