Warthin's Tumors

Findings:

A well-defined complex cystic and solid mass involves the lower margin of the right parotid gland. A smaller nearly solid mass is seen within the left parotid gland. No adenopathy is included.

Discussion/Differential Diagnosis:

The primary differential diagnosis for complex cystic appearing bilateral neck masses in a middle aged to elderly adult is nodal metastases from squamous cell carcinoma, therefore the site of lesions should be confirmed as intraparotid and not extraparotid. Intraparotid metastasis from a head and neck mucosal site can occur, but would be unusual to be isolated to the parotids and be bilateral without a visible primary site. Warthin's tumors should be considered for bilateral complex cystic intraparotid masses in the absence of any visible mucosal mass. Other examples and discussion:
