Motor Strip Infarction
Findings:
Axial diffusion, ADC, FLAIR and ASL perfusion images demonstrate ribbon like restricted diffusion involving the right frontal parietal cortex along the expected motor strip. Subtle FLAIR hyperintensity is present in this region. The restricted diffusion is limited to the overlying cortex. The ASL images demonstrate hyperperfusion symmetrically involving the bilateral anterior parietal lobes, posterior to the motor strip.
Discussion/Differential Diagnosis:
The differential diagnosis for this process includes infarction and seizure related signal abnormalities. Focal encephalitis is less likely. It illustrates the anatomic position of the central sulcus which is lined by the primary motor cortex.
