DNET

Findings:

Multiple MR images demonstrate clustered rounded irregular T2 hyperintensities within the cerebellar vermis which demonstrate increased signal on FLAIR imaging, with no abnormal enhancement or significant mass effect. There is no diffusion restriction or hemorrhagic staining in this region.

Differential Diagnosis:

While posterior fossa neoplasms have a broad differential diagnosis that depends on age of the patient and clinical/imaging features, the lack of surrounding edema, absent enhancement, and no mass effect indicate a lower grade lesion. Some of the sequences simulate the appearance of prominent perivascular spaces, however the small cystic lesions are FLAIR hyperintense. The cluster of grapes appearance is somewhat characteristic for DNET. An unusual hamartomatous proliferation might appear similar.
