Right Transverse Sinus Thrombosis due to Occipital Bone Fracture

Findings:

Noncontrast CT images demonstrate asymmetric hyperdensity of the right transverse sinus and a non-displaced right occipital bone fracture. The CT venogram confirms that the hyperdensity is related to complete occlusive thrombosis of the right transverse and sigmoid sinuses.

Differential Diagnosis/Discussion:

Asymmetric hyperdensity of a vascular structure on CT should raise the possibility of underlying thrombosis. More commonly though, all vascular structures are symmetrically hyperdense on CT due to either hemoconcentration or recently administered IV contrast. Polycythemia with elevated red cell mass can also cause diffuse vascular CT hyperdensity. Dural sinus thrombosis is not uncommon in the presence of occipital bone fractures.
